Free Microchip Clinic presented by Gerald and Diane Roba

Join us on Saturday, September 18th from 9 am - 12 pm at LCHS for a Free Microchipping event.

To be eligible for the Free Microchip you must meet the requirements below:

Your pet must be on a leash
You must fill out the form below
You must register on the Home Again website
You must bring proof of vaccinations for your pet
You must bring proof of 2021 animal license
You must bring proof of your identification (drivers license or state ID)
If your animal is anxious around people or other animals they need to be muzzled for their safety and the safety of others.
